Instructions for Submitting the
Assessment of Competencies Form
The Assessement of Competencies Form is used by participants of the certification program for identifying competencies that have been achieved as well as those which still need to be acquired. A blank copy of the form may be found here.
Following are a list of instructions for completing the Assessment of Competencies Form. Directions are given both for participants who already have a specific competency and for those participants who need to acquire a specific competency. Examples are provided throughout. A red font is used in the examples to highlight information that is entered for the specific step that is being explained.
When the participant already has a competency:
The top half of the Assessment of Competencies worksheet is completed listing:
- The specific competency that has already been attained
- Practical examples demonstrating how the competency has been used
- The name of a supervisor or mentor who has witnessed this competency in the participant
- The date which the competency was achieved
- The dated signatures of both the participant and the supervisor/mentor.

When the participant needs to acquire a new competency:
A 5-step process is followed in order to complete the Assessment of Competencies form:
- The participant begins with the specific competency which needs to be acquired. The learning activities that will be used to help attain the competency are also listed. Example.
- A Personal Development Plan is completed, listing the specific actions that are planned by the participant for acquiring the competency. Example.
- Upon completing specific learning activities (e.g., workshops, formal courses), the participant fills out the remainder of the Assessment of Competencies form. Learning activities are validated by attaching records of the workshops/activities that were attended. Dates for the workshops/activities are inserted, as are the number of credit hours that were earned by attending the workshop or activity. Example.
- When the participant has demonstrated that he/she has achieved a new competency, the column "How I reflect this competency in my work" is completed. This includes specific details of how the competency has been used by the participant. It also includes validation of the competency from a supervisor or mentor who has witnessed the demonstration of the new competency in the participant. Example.
- At this point, the form is signed and dated by both the participant and the supervisor or mentor.
